Shogun Cagefighting Championships, LLC Fight Contract

This agreement made and entered into this ____ day of ______________, 20___ between Shogun Cagefighting Championships, LLC, of the city of Lebanon, in the state of Tennessee, a mixed martial arts promoter, herein after called "promoter", and _________________ (Fighter’s Name) of the state of ____________, herein after called, "fighter", follows:

1. The fighter agrees to appear and engage in an amateur mixed martial arts contest with __________________ (Opponent’s Name), scheduled for 3 rounds of 3 minute duration for a non-championship bought, or 3 rounds of 3 minute duration for championship bouts. This bout will be held at a weight of _______ lbs. The bout will be held on ______________________, 20____, at _______________________ (Name of Venue), in _____________________ (Town), __________ (State).

2. "Fighter" agrees his participation in the bout shall be an honest exhibition of his abilities as a mixed martial artist.

3. "Fighter" understands there is an “assumed risk associated” with mixed martial arts, and understands mixed martial arts, is a contact sport and in the event of physical injury or personal loss occurs will hold harmless promoter and its employees.

4. If "fighter" fails to show up for scheduled bout, fighter will be held financially responsible by "promoter". A “no show” fee of $25.00 will be charged to “fighter” with payment due to “promoter” within 10 days of original bout date listed here in. If “fighter” neglects to pay said fee he shall be barred from future participation in Shogun Cagefighting Championships, LLC events.

The following circumstances may be an excuse for breaking this contract, but you must provide written documentation of occurrence; death or grave injury/illness of immediate family member, activation to military service, an injury that bars you from competing.

*Immediate family member is defined as a spouse, sibling, child, parent or grandparent.
*Injuries must be documented by a physician’s statement prior to the bout.

BY SIGNING BELOW, ALL PARTIES AGREE THIS IS A LEGAL AND BINDING CONTRACT.
